The iTrak Workplace Safety & Insurance Board (WSIB) of Ontario Reporting Module provides an integrated forms management solution that allows for additional incident reporting to WSIB of Ontario standards. The supported WSIB forms include: WSIB Form 6 (Worker's Report of Injury/Disease), Form 7 (Employer’s Report of Injury/Disease) and the WSIB FAF Form (Functional Abilities Form for Early and Safe Return to Work).


An employee related incident can now be automatically configured to attach the required WSIB form(s) while managing the data collection process. By having the WSIB forms entered, generated and printed within the iTrak Incident Reporting & Risk Management Platform, safety and HR departments will benefit from efficient and timely management and report generation to the Worker’s Safety & Insurance Board of Ontario.
